So CSLookup setup is ok.
Now if selected values are not saved, you should check the field types (CSLookup can't do nothing except set field values). Check which CB field type are you using and disable any field validation. Let me know...

field type..??
it says - "drop down, single select".
country i just us txt field.
looks all ok on the front end, it just isnt saving.. uhm..

Ok,
choose "text field" and leave "Field-specific Parameters" as default. Is the CSLookup Javascript that changes the type into drop down single select field. It should work...
